Open Appointment Cards

For issue to in-patients at the point of discharge

Open Appointment Cards are issued by clinics to patients at the point of discharge. Prior to issue, the card is completed with the relevant specialty, a specified period of time, the date of discharge and a contact telephone number which the patient can call, should they experience any problems during the period specified.

Issue of the Open Appointment Card provides every discharged patient with the important assurance that despite their treatment having been completed successfully, the hospital and staff will continue to care for their welfare and will be prepared to assist them should the need arise.
